Medical Statement for Children with Special Dietary Needs

Each special dietary request must be supported by a statement that explains the food substitution that is requested. It must be signed by a recognized medical authority. In these cases, recognized medical authorities may include physicians, physician assistants or advanced nurse practitioners.

The medical statement MUST include:

  • An identification of the medical or other special dietary condition that
    restricts the child’s diet
  • The food or foods to be omitted from the child’s diet
  • The food or choice of food to be substituted

In Case of Food Allergies

Generally, children with food allergies or intolerances do not have a disability as defined under either Section 504 of the Rehabilitation Act or Part B of IDEA. Child Nutrition may, but is not required to, make food substitutions for them. If a student has a severe life threatening food allergy, please contact the Director of Food Services Kim Carter to discuss menu options or substitutions.

To Request a Special Diet

  1. Obtain a Special Dietary Needs Application from school Cafeteria Manager or school nurse or download below and complete information.
  2. Get licensed Physician or medical authority to complete the form and sign.
  3. Submit the completed form to the school cafeteria manager or Food Service office at 305 Baker St.
