Medical Statement for Children with Special Dietary Needs
Each special dietary request must be supported by a statement that explains the food substitution that is requested. It must be signed by a recognized medical authority. In these cases, recognized medical authorities may include physicians, physician assistants or advanced nurse practitioners.
The medical statement MUST include:
- An identification of the medical or other special dietary condition that
restricts the child’s diet - The food or foods to be omitted from the child’s diet
- The food or choice of food to be substituted
In Case of Food Allergies
Generally, children with food allergies or intolerances do not have a disability as defined under either Section 504 of the Rehabilitation Act or Part B of IDEA. Child Nutrition may, but is not required to, make food substitutions for them. If a student has a severe life threatening food allergy, please contact the Director of Food Services Kim Carter to discuss menu options or substitutions.
To Request a Special Diet
- Obtain a Special Dietary Needs Application from school Cafeteria Manager or school nurse or download below and complete information.
- Get licensed Physician or medical authority to complete the form and sign.
- Submit the completed form to the school cafeteria manager or Food Service office at 305 Baker St.
