Voluntary Student Accident Insurance

San Angelo ISD offers parents the opportunity to purchase voluntary accident insurance coverage for their child or children.This voluntary insurance pays benefits before any other insurance coverage pays. Our plans offered are: school-time coverage, with or without Interscholastic sports and UIL activities, full-time coverage, with or without Interscholastic sports and UIL activities, voluntary Varsity football coverage, and Dental coverage. To learn more more about our voluntary plan benefits, please download one of the brochures below. Plan brochures are also available at your campus office. SAISD Extra-Curricular Transfers

Beginning in the Fall of 2017, a San Angelo ISD student’s residency for extra-curricular eligibility will be determined based upon the campus the student enters or has entered during his or her 9th grade year. That campus will be considered the student’s home campus. If a student wishes to transfers from one high school to another in SAISD and continues to reside in the same attendance zone, the student will not be eligible to participate in athletics for the remainder of the time that he or she is in high school. Therefore, if a student desires to live in one attendance zone and transfers to the other, he or she will not be eligible for UIL activities. Click the links below for more information.

Online Athletic Forms

All required athletic paperwork except the Pre-participation Physical and Medical History (see below) are filled out electronically. In order to submit online paperwork appropriately, the parent/guardian will be asked to provide information such as student ID number, home phone numbers, cell phone numbers, family insurance, email addresses, emergency contact names and other pertinent information.

To submit athletic forms electronically:

  1. Please go to to submit athletic electronic forms.
  2. Go to Register Now and register for your account.
  3. Go to electronic Forms
  4. Fill out electronic Forms

You will need both parent and student available at the same time to finish forms. All spaces MUST be filled out – if blank does not pertain to you - put “none” in the space. Ex: If you do not have a home phone number put “none” in the space.

Athletic Physicals

SAISD requires all athletic participants to obtain a yearly physical examination prior to any participation in games, practices, try-outs, or workouts (in-season or out-of-season).

The required Pre-participation Physical Evaluation and Medical History forms cannot be filled out online because it must be completed by a local physician. Students are required to use the Pre-participation Physical Evaluation and Medical History forms provided by SAISD. No other Physical Examination Form can be accepted. All physicals must be dated after May 1st to be valid for the upcoming school year.

Physical forms are color-coded and dated annually for documentation and filing purposes. Parents/Guardians may obtain the correct form from the high schools, middle schools, or from the athletics office at the SAISD Administration Building at the beginning of the school year.
